Who Needs Immunizations?

All children ages 0-5 years of age.  School age children provide them to their public school.

 

Why are Immunizations Required? 

Proof of immunization is a law required by the State of Utah in every school and childcare facility in the state. 

 

How To Get Us Your Immunizations?

Choosing Not to Immunize?

In order to be exempt from immunizations, you’ll need to do one of the following:

• Obtain a certificate from a licensed physician stating that due to the physical condition of the student one or more specified immunizations would endanger the child’s life or health

• A completed health form from the local health department where student lives, witnessed by local health officer

• A statement that the student is a bona fide member of a religious organization whose teachings are contrary to immunizations, signed by legal parent or guardian
